The client understand that the fitness/exercise training provided by Erik Bickerstaff  2 High Fives (Head Fitness Trainer/Sports Coach) is intended to enhance my physical and mental well being including body composition, cardiovascular endurance, muscle strength, muscle endurance and flexibility.  Any other intended purposes for the fitness/exercise training are specified below. 

The general benefits of fitness/exercise training and possible injuries or contraindications have been explained to me.  I understand that any type of fitness and/or exercise is not a substitute for medical treatment or medications, and that it is recommended that I concurrently see my Primary Care Physician for any condition(s) that I may have currently or will have in the future.  I am aware that a fitness/exercise trainer does not diagnose illness or disease, or injuries.   List any injuries or medical issues that are current or in the past

2 High Fives does not prescribe medications, meal plans, or advice to injury care.  Any injuries that may occur when working out with a trainer at 2 High Fives is not responsible for any fault or medical liability that is sustained at time of work out or anytime while training with a trainer.   


I have informed the fitness trainer/sports coach of all my known physical conditions, medical conditions and medications, and I will keep the trainer updated on any changes to my health that may occur in the future.  










I understand that neither 2 High Fives nor Erik Bickerstaff is responsible for the aggravation/injury of any conditions that were present or may occur while training.
